PRIMARY MEMBER DETAILS
RIDER NAME: Enter the rider name
RACE RESULT TRANSPONDER No. The transponder number is 5 letters followed by 2 numbers. If you don't have Race Result transponder enter X.
DATE OF BIRTH : Enter your date of birth in dd/mm/yyyy format
MEMBERSHIP TYPE : Expert Championship (A-Group only)
Clubman Championship (A,B,C Groups)
Vets Rider (over 35 years)
Vets Rider (over 50 years)
Rookie Rider (14 to 17 years)
85cc Rider Small Wheels (9 to 12 years)
85cc Rider Big Wheels (11 to 15 years)
 Indicate the type of membership required.
Refer to the Rules and Regs for full details.
PREFERRED RACE NUMBERS: Enter your preferred race number followed by a second choice. For example 27, 272.

RIDER NAME: Enter the rider's name. Enter X if no secondary member is required.
RACE RESULT TRANSPONDER No. Enter the transponder number. Enter X if you don't have Race Result transponder or if no secondary member is required.
DATE OF BIRTH : Enter the rider's date of birth in dd/mm/yyyy format
MEMBERSHIP TYPE : Not Required

PAYMENT DETAILS
PAYMENT METHOD: Bank Transfer
Bank Transfer is the only payment method and you will receive an email with the club's bank account details.
MEMBERSHIP FEE TO PAY: £50 All Solo Memberships
£90 Combined Family Membership 		Indicate the amount to for your membership(s).

Under 18s please ensure your parent or guardian completes this membership form and note a Parental Declaration form will need to be completed and signed at each event.
